• Supplies can ony be issued with a current prescription on file.  Prescriptions are good for one year after set up.
• We can request an updated prescription when needed.
• Pick up of items is avialable during office hours. Monday-Friday 8:30am-4:45pm.
• Items may be shipped at the patients expense.  Insurance will not reimburse for shipping.
• Please send in a supply request via email or call  the office. Your items will then be ready when you arrive.
• Insurance billing will be handled by our office.  Please assist us with any insurance or physcian changes.
